Do you need a new church name tag? Please fill out this form for each person who needs a name tag.
We place name tag orders periodically, and will notify you when your name tag is ready for pick up.
We offer name tags to new members at no cost. If you need a replacement name tag and would like to help cover the cost (approximately $8 per name tag) you can include it in your contribution to our general operating fund. No special designation is needed!
